Dual Eligible Special Needs Plan (D-SNP)

What is a D-SNP plan and what does it include?


With the D-SNP (HMO) plan, Medicare and Medicaid benefits are rolled into one simple health plan, and members don’t pay anything for covered medical services. How much Medicaid covers depends on the member’s income, resources and other factors. Those who have full Medicaid benefits may enroll in this plan.

Part D adds prescription drug coverage to:


✔ Original Medicare
✔ Some Medicare Cost Plans
✔ Some Medicare Private-Fee-for-Service Plans
✔ Medicare Medical Savings Account Plans

These plans are offered by insurance companies and other private companies approved by Medicare. Medicare Advantage Plans may also offer prescription drug coverage that follows the same rules as Medicare Prescription Drug Plans.
